About the role
Key responsibilities & impact- Deliver hospital-level rehabilitation in patients' homes
- Manage an autonomous caseload while working with a multidisciplinary team
- Conduct comprehensive physiotherapy assessments in patients' homes
- Provide evidence-based rehabilitation programs supporting recovery, mobility and independence
- Support patients recovering from elective orthopaedic surgery and acute hospital admissions
- Develop personalised treatment plans aligned with individual goals and outcomes
- Use virtual care and digital health technologies to enhance engagement and continuity of care
- Collaborate with medical, nursing and allied health professionals
- Support early discharge and hospital avoidance initiatives
- Maintain accurate clinical documentation and reporting
- Contribute to quality improvement and service development
- Champion patient-centred, evidence-based rehabilitation practice
Requirements
What you’ll need- Current registration as a Physiotherapist with AHPRA
- Minimum three (3) years' post-graduate physiotherapy experience
- Experience in rehabilitation, orthopaedics, subacute care or community-based services highly regarded
- Strong clinical reasoning and evidence-based decision-making skills
- Excellent communication, organisation and time management skills
- Confidence working independently in community settings
- Experience with digital health platforms, telehealth or virtual care models, or willingness to embrace new technologies
- Current Australian Driver's Licence
- Access to a reliable motor vehicle
- Satisfactory pre-employment checks, including professional registration verification, immunisation requirements and relevant probity screening
