About the role
Key responsibilities & impact- Deliver evidence-based physiotherapy assessments, interventions and exercise prescription for people living with heart failure
- Conduct virtual assessments via phone and video platforms
- Use digital health technologies for exercise prescription, monitoring and clinical support
- Monitor patient outcomes and clinical risk, escalating concerns and collaborating with the broader care team
- Facilitate patient-centred goals supporting health, wellbeing and self-management
- Collaborate with cardiologists, nurses, care coordinators, hospital teams and primary care providers
- Support early discharge and reduce avoidable hospital admissions
- Lead physiotherapy service development and contribute to the Hearts at Home model of care
- Participate in quality improvement, service evaluation and program development
- Provide expert advice, mentoring and supervision to colleagues
- Maintain clinical documentation, activity reporting and regulatory compliance
- Promote the Hearts at Home program and support stakeholder engagement
- Participate in orientation, education and professional development activities
Requirements
What you’ll need- Current registration as a Physiotherapist with AHPRA
- Minimum five years' post-registration experience, including at least three years in cardiac and/or cardiopulmonary rehabilitation settings
- Highly developed clinical assessment, rehabilitation and exercise prescription skills in cardiac and/or heart failure settings
- Telehealth or virtual care assessment and rehabilitation experience highly regarded
- Strong clinical reasoning and evidence-based decision-making skills
- Expertise interpreting medical management, diagnostic investigations, treatment plans, medications and biometric observations
- Excellent communication and stakeholder engagement abilities
- Ability to work autonomously and effectively within multidisciplinary teams
- Experience leading service improvement, quality initiatives or program development
- Experience supervising, mentoring or supporting other clinicians
- Strong organisational, documentation and time-management skills
- Commitment to patient-centred care and continuous quality improvement
- Satisfactory pre-employment checks, including professional registration verification, immunisation requirements and relevant probity screening
