About the role
Key responsibilities & impact- Provide direct patient care and skilled tasks under the direction of a Physician, Advanced Practitioner, or Registered Nurse
- Collaborate with the treatment team
- Facilitate communication among healthcare team members, patients, and families
- Participate in interdisciplinary team and rehab meetings
- Administer treatments and medications according to physician’s orders
- Record treatments and medications accurately and promptly
- Document resident/patient care and responses or lack of responses to interventions
- Report changes in resident/patient condition to the RN
- Assist with admission, transfer, and discharge of residents/patients
- Demonstrate the Network’s Service Excellence Standards in interactions with internal and external customers
- Comply with Network and departmental attendance and dress-code policies
Requirements
What you’ll need- Graduate of an accredited Pennsylvania nursing school for LPN
- Current Pennsylvania LPN license
- Current BLS certification
- New or experienced candidates accepted
- Ability to work holiday and weekend requirements per unit and company policies
- Ability to participate in interdisciplinary team and rehab meetings
- Ability to administer treatments and medications accurately and timely according to physician’s orders
- Ability to document patient care, treatments, medications, and responses to interventions
- Ability to report changes in patient condition to the RN
- Ability to assist with admissions, transfers, and discharges
- Ability to meet physical and sensory requirements, including lifting, carrying, pushing, pulling, patient transport, bending, kneeling, climbing, reaching, and visual requirements
