About the role

Key responsibilities & impact
  • Promote, maintain and improve individual and community health within SLUHN’s population of covered lives
  • Serve as a health coach for a designated group of employees and/or spouses
  • Initiate contact, assist with care navigation, connect individuals to programs and services, and follow through for compliance
  • Provide individual or group counseling and education to patients
  • Identify barriers and develop treatment plans based on individual needs related to health education programs
  • Act as a resource and change agent to improve patients’ health status
  • Monitor health education programs for effectiveness
  • Maintain complete and accurate patient documentation for follow-up and program data reporting
  • Collect and record data in corresponding databases and/or electronic medical records
  • Design, implement and evaluate health education materials/modules as needed
  • Support employee completion of health assessment requirements
  • Participate in assigned committees
  • Maintain confidentiality and comply with Network, departmental, safety, reporting, attendance and dress code policies
  • Demonstrate Service Excellence, performance improvement and financial responsibility

Requirements

What you’ll need
  • Clinical license such as RN or RD
  • At least 2 years of health coaching or health education experience
  • Proficient in Microsoft Office: Word, Excel, PowerPoint, Access and Publisher
  • Excellent oral and written communication skills
  • Ability to engage and maintain constructive relationships with a diverse population
  • Ability to maintain confidentiality
  • Ability to meet the stated physical and sensory requirements
  • Availability for some evenings and/or weekend events
  • Motivational interviewing experience preferred
  • Previous experience working with employee populations preferred
  • Bilingual Spanish preferred
  • MPH with coaching credential or Certified Health Education Specialist (CHES) preferred
  • Experience designing health education materials preferred
