About the role

Key responsibilities & impact
  • Provide comprehensive case management for patients admitted to the inpatient acute rehabilitation unit from admission through discharge
  • Secure coverage through continued-stay reviews, retro authorizations, pre-certifications, and other authorization efforts
  • Assess, plan, implement, coordinate, and evaluate health and community services addressing patients’ physical, psychosocial, vocational, and health needs
  • Perform admission reviews and identify psychosocial barriers, interventions, and discharge-planning needs
  • Conduct ongoing discharge-planning assessments, mitigate barriers, and facilitate safe, timely, efficient discharges
  • Collaborate with patients, families, caregivers, and interdisciplinary teams to design individualized discharge plans
  • Maintain contact with patients and families at least three times weekly
  • Participate in daily interdisciplinary stand-up meetings and weekly interdisciplinary team meetings
  • Arrange community, state, and federal services and durable medical equipment before discharge
  • Provide assistance with medical paperwork and educate patients and families about diagnosis, treatment, adaptation, discharge plans, and life situations
  • Collaborate with outpatient Care Managers for handover and post-discharge follow-up
  • Serve as a resource to staff regarding coverage issues, discharge options, community resources, regulatory issues, and psychosocial processes
  • Educate patients about patient choice, medical treatment, advance directives, and related rights
  • Maintain timely clinical records documenting patient contacts, reviews, interventions, recommendations, and referrals

Requirements

What you’ll need
  • MSW, RN, or master’s degree in a related field of physical, occupational, or speech therapy
  • BSN preferred for RN candidates
  • At least two to five years of clinical experience in healthcare preferred, preferably in a rehabilitation hospital
  • Knowledge of care management and discharge planning
  • Ability to assess psychosocial barriers and coordinate discharge services
  • Ability to facilitate authorizations, pre-certifications, concurrent reviews, and retro authorizations
  • Ability to maintain clinical records according to hospital policy and Medicare, CARF, and JCAHO guidelines
  • Ability to communicate with patients, families, caregivers, and interdisciplinary teams
  • Ability to use documentation and clinical record systems
  • Ability to meet the stated physical and sensory requirements, including sitting, standing, walking, writing, typing, and lifting up to 10 pounds
  • Application must use full legal name and current home address
  • Employment history for the past seven years, including present employer, must be provided
