About the role
Key responsibilities & impact- Conduct virtual wellness visits directly to patients in their homes
- Review case load and prepare technology for patient service
- Navigate daily schedule using modern technology
- Engage in proactive care by conducting health risk assessments and medication reviews
- Work with Sprinters to evaluate vital signs, arrange blood draws, and perform necessary tasks
- Provide care coordination and management including preventive care interventions, medication management, and documentation
Requirements
What you’ll need- Board Certified as a Family Nurse Practitioner
- Active Family Nurse Practitioner License in California
- Credentialed Medicaid provider for the state of California (Medi-Cal)
- Consistently exhibits the highest levels of professionalism, integrity, accountability, confidentiality, care and compassion to provide high quality health services
- Willingness to work in a revolutionary environment that sometimes necessitates last minute problem solving and out of the box thinking
- Technologically savvy and comfortable using tools such as laptops or mobile devices for charting and HIPAA secure messaging apps for care coordination
- Strong written and verbal communication skills
- Ability to work independently or in a team environment
- Pass national background check and valid clinical license search
