About the role
Key responsibilities & impact- Conduct virtual wellness visits directly to patients in their homes
- Perform health assessments and a variety of healthcare services
- Review case load and prepare technology for the day
- Collaborate with clinical in-home team members to visit each patient's home
- Conduct thorough health risk assessments and medication reviews
- Engage in proactive care and educate patients on health and well-being
- Provide comprehensive care coordination and management
- Arrange blood draws and evaluate vital signs as necessary
Requirements
What you’ll need- Board Certified as a Family Nurse Practitioner
- Active Family Nurse Practitioner license in Louisiana
- Consistently exhibits the highest levels of professionalism, integrity, accountability, confidentiality, care and compassion to provide high quality health services
- Willingness to work in a revolutionary environment that sometimes necessitates last minute problem solving and out of the box thinking
- Technologically savvy and comfortable using tools such as laptops or mobile devices for charting and HIPAA secure messaging apps for care coordination
- Strong written and verbal communication skills
- Ability to work independently or in a team environment
- Pass national background check and valid clinical license search
