About the role
Key responsibilities & impact- Conduct virtual wellness visits directly to patients in the comfort of their homes.
- Leverage medical expertise to offer a wide range of healthcare services.
- Make warm and welcoming connections with a diverse range of patients.
- Engage in proactive care by conducting health risk assessments and educational informational sessions.
- Provide comprehensive care coordination and management, including preventive care interventions and medication management.
Requirements
What you’ll need- Board Certified as a Family Nurse Practitioner
- Active Family Nurse Practitioner license in any of the following states: KY, MI, OH, IL
- Consistently exhibits the highest levels of professionalism, integrity, accountability, confidentiality, care and compassion to provide high quality health services
- Willingness to work in a revolutionary environment that sometimes necessitates last minute problem solving and out of the box thinking
- Technologically savvy and comfortable using tools such as laptops or mobile devices for charting and HIPAA secure messaging apps for care coordination
- Strong written and verbal communication skills
- Ability to work independently or in a team environment
- Pass national background check and valid clinical license search
