About the role
- Responsible for overall operation of case management and social services activities
- Performs concurrent patient assessments related to appropriateness of care
- Provides consultations to medical staff and interdisciplinary team members
- Documents discharge planning services professionally and collaboratively
- Facilitates care conferences with patients and families
Requirements
- Master’s Degree in Social Work or Counseling
- LCSW or LCPC in Illinois
- 2 years of experience in providing individual, family and/or group therapy
- Experience in addiction disorders/cross addictions
- Excellent interpersonal skills
- Excellent knowledge of available community resources
