RN Care Manager
Shriners Children'sRN Care Manager at Shriners Children’s overseeing assessments, planning, and follow-up for pediatric patients and their families. Ensuring continuity of care within a multi-disciplinary team for positive outcomes.
About the role
Key responsibilities & impact- Provides assessments, planning, interventions and follow-up for patients and family members
- Coordinates the plan of care to achieve desired outcomes and integrates the work of the health care team
- Ensures the development of a safe and effective plan of care through early identification and assessment of the patient's needs
- Collaborates with patients, families, caregivers, and the health care team for optimal outcomes
- Arranges services among community agencies, physicians, patients, family, caregivers, and others involved in the plan of care
- Ensures prudent utilization of all resources by evaluation of the resources available to the patient
Requirements
What you’ll need- BSN RN license in state of practice
- Case Management Certification (CMC, ACM or ANCC), or willingness to achieve within 12 months of eligibility
- 3 years Nursing experience
- Preferred MSN
- 5+ years of clinical experience in Pediatrics, Burn care and/or Orthopaedics
- 3-5 years Care Coordination/Case Management experience
- Experience with utilization review/management with a working knowledge of InterQual criteria
