About the role
- Provide physical therapy services of consultation, evaluation, and direct treatment
- Perform accurate and timely initial and discharge evaluations and assessments of progress
- Implement a comprehensive and individualized treatment program for a diverse patient population
- Provide direct Physical Therapy Services including strength, balance, flexibility, endurance, gait, transfer and mobility training
- Conduct clinic screening and discuss findings with the patient, family, and physician
- Initiate discharge equipment needs for inpatients in conjunction with the Care Coordination Department
- Provide evaluations of and recommendations for equipment
- Provide timely and accurate documentation
Requirements
- Bachelors degree in Physical Therapy
- Illinois PT license
- One year rehab/therapy experience
