About the role

Key responsibilities & impact

Provide comprehensive medication management, including reconciliation, dose optimization, adherence support, and drug-level assessments aligned with evidence-based standards
Partner with physicians, specialists, and interdisciplinary teams to order individualized care plans under a Collaborative Practice Agreement (CPA) and promote seamless medication coordination
Monitor therapy effectiveness and safety, identify optimization opportunities, and intervene as appropriate
Support quality-improvement and REMS initiatives
Support teammates with complex prior authorizations, improve workflows, educate patients, and maintain ongoing clinical development
Build relationships with patients and providers through virtual patient assessments and evidence-based treatment-plan monitoring

Requirements

What you’ll need

Hold a Doctor of Pharmacy (PharmD)
Must be a licensed pharmacist in Washington
Completion of a PGY2 Ambulatory Care Residency, or completion of a PGY1 Residency and at least 1 year of ambulatory practice under a Collaborative Practice Agreement (CPA), or at least 3 years of ambulatory practice with disease state management
Strong communication, organization, and tech skills
Empathy, integrity, and a team-first mindset
Solid computer skills required, including proficiency in Microsoft Office tools (Outlook, Teams, Word, Excel, and PowerPoint)
Ability to navigate the internet and web-based platforms
Data entry and computer skills will be evaluated through a pre-employment assessment
