About the role

Key responsibilities & impact
  • Review clinician patient-care notes and assessments for prior authorization data and care-plan recommendations
  • Develop and refine disease- and drug-specific clinical protocols
  • Monitor patient adherence across sites and specialty disease categories, especially oncology
  • Evaluate EMR data, drug and disease interactions, medication selection, labs, allergies, prescriptions, renewals, and related clinical information
  • Recommend therapeutic regimens and disease-specific treatments according to guidelines
  • Monitor therapies for side effects and medication-related issues and recommend changes to prescribers
  • Perform medication therapy management, document interventions, and assess adherence rates
  • Conduct pharmacist consultations, motivational interviewing/coaching, medication reconciliation, regimen optimization, synchronization, reminders, and care coordination
  • Monitor productivity and report weekly growth statistics
  • Oversee technicians' daily activities and verification accuracy
  • Identify and resolve insurance issues and financial-assistance opportunities
  • Supervise support-personnel communications in compliance with HIPAA rules
  • Maintain pharmacist registration and remote pharmacist activity in compliance with applicable pharmacy laws and DEA requirements
  • Communicate directly with other health professionals regarding patient care
  • Train pharmacy staff, students, interns, externs, and healthcare professionals
  • Manage and update relevant data using database applications
  • Support operational improvements, productivity, quality, and customer-service standards
  • Assist with research projects and peer review
  • Identify trends, resolve problems, recommend improvements, and implement change
  • Participate in process-improvement initiatives

Requirements

What you’ll need
  • Must hold a Doctor of Pharmacy (PharmD) or equivalent advanced degree
  • PharmD or equivalent advanced degree required
  • Must be a licensed Pharmacist
  • NH pharmacist licensure required by the role title/Job Location metadata
  • Maintain current registration and comply with local, state, and federal pharmacy laws and DEA regulations
  • 1+ years' experience strongly preferred but not required
  • Deep knowledge of specialty medication pharmacology
  • Knowledge of local, state, and federal pharmacy regulations
  • Strong technical and clinical skills
  • Ability to analyze information and exercise good judgment
  • Strong verbal and written communication skills
  • Ability to manage processes and people
  • Proficiency with Microsoft Word, Excel, and PowerPoint
  • Computer literacy and working knowledge of database and reporting tools
  • Solid computer skills, including Outlook, Teams, Word, Excel, PowerPoint, internet, and web-based platforms
  • Data-entry and computer skills are evaluated through a pre-employment assessment
  • Ability to identify barriers to medication adherence and implement and monitor improvement plans
  • Ability to motivate and coach patients toward better outcomes and therapy goals
  • Knowledge of complex/chronic disease states and specialty-pharmacy therapies preferred, including oncology, transplant, hepatitis C, infectious disease/HIV, multiple sclerosis, rheumatoid arthritis, GI/IBD, cystic fibrosis, psoriasis, hyperlipidemia, lupus, and other inflammatory conditions
  • Organized, high-integrity, detail-oriented, dependable, quality-focused, empathetic, and an effective listener/communicator
  • Energetic, highly motivated team player with strong interpersonal and communication skills
  • Discretion and confidentiality when handling sensitive and private data
  • EPIC/QS1/WILLOW knowledge is a plus, not required
