About the role

Key responsibilities & impact

Provide comprehensive medication management, including reconciliation, dose optimization, adherence support, and drug-level assessments aligned with evidence-based standards.
Partner with physicians, specialists, and interdisciplinary teams to order individualized care plans under a CPA and promote seamless medication coordination.
Monitor therapy effectiveness and safety, identify optimization and intervene as appropriate.
Support quality-improvement and REMS initiatives.
Provide recommendations to operations by supporting teammates as needed with complex prior authorizations, improving workflows, educating patients, and maintaining ongoing clinical development.

Requirements

What you’ll need

Hold a Doctor of Pharmacy (PharmD)
Must be a licensed pharmacist in Oregon
Certified as an HIV Specialist (AAHVIP)
Completion of a PGY2 Ambulatory Care Residency or PGY1 Residency and at least 1 year of ambulatory practice under a CPA or at least 3 years of ambulatory practice with disease state management
Strong communication, organization, and tech skills
Empathy, integrity, and a team-first mindset
