About the role
- Lead the shelter case management team to provide comprehensive services to households experiencing homelessness
- Carry a caseload at the off-site hotel location and provide direct case management
- Develop and implement individualized housing-first service plans to secure and maintain permanent housing
- Facilitate housing-related tasks including collecting documents and preparing rental assistance packages
- Assist with developing rental assistance packages, document fund distribution, and lead recertification processes
- Collaborate with community partners, service providers, hotel staff, LHSC site, and Assistant Director of Programs
- Supervise, support, and provide guidance to Case Managers, including phone screenings and diversion support
- Ensure accurate and timely HMIS data entry, monthly data clean-ups, and oversee direct reports' data entry
- De-escalate safety concerns and complete incident documentation before shift end
- Participate in intakes, on-call manager rotations, staff meetings, trainings, and file audits
- Manage tasks and scanning to maintain complete and current participant charts
- Facilitate ongoing reviews of service plans and lead meetings and supervision sessions as needed
Requirements
- A bachelor's degree in human services/ related field or commensurate experience
- 3+ years of street outreach/case management/ direct services/other related experience
- Strong written and oral communication skills
- Ability to prioritize competing priorities and make sound judgements
- Ability to complete tasks while navigating frequent interruptions
- Ability to deliver creative, resourceful solutions to unique challenges
- Strong organizational and time management skills
- Valid Driver’s License and reliable transportation
- Ability to Drive a 15-passenger van
- Willingness to work evenings and weekends and as needed
- Annual TB Test is required
- Ability to sit or stand for long periods
- Ability to lift items weighing 10-20 pounds
