About the role
Key responsibilities & impact- Oversee 15 direct reports
- Serve as first-line supervisor in the Department of Medical Care Management for the assigned site/function
- Facilitate the work of assigned team members
- Lead ongoing case manager competency assessment, needs identification, and educational offerings
- Provide educational services to Medical Care Management staff
- Participate in assigned team activities and provide case management services as needed
- Direct daily operations, including coordination of work, quality, and service
- Improve health outcomes through Sentara Health Plan’s care management and health plan services
Requirements
What you’ll need- Bachelors Degree in Nursing REQUIRED
- 3 years experience in Case Management REQUIRED
- Registered Nurse (RN) License (Compact or Virginia) preferred
- Specialty certification required within one year of eligibility (ACM, CCM, CCCTM or RN-BC)
- Supervisory experience preferred
- Extensive Nursing Facility or Long Term Care Experience preferred
- Knowledge of Long-Term Services and Supports (LTSS) / Home and Community-Based Services (HCBS) or Medicaid requirements
- Ability to work 8am to 5pm EST, Monday through Friday
- Remote work available only in specified states
