About the role

Key responsibilities & impact

Use critical thinking skills and respond quickly and professionally under stressful and emergent situations
Provide clinical assessment and nursing triage to diverse patients across the health continuum
Provide health education to culturally diverse populations
Direct patients to the most appropriate level of care
Provide telephonic health information and nursing education
Empower patients to make better healthcare decisions
Help patients participate in external programs and available resources
Collaborate with Telehealth providers by referring appropriate patients for virtual care visits
Provide outbound follow-up calls as necessary
Perform other duties as assigned

Requirements

What you’ll need

Nursing Degree (Diploma, ADN, BSN) required
Active, unrestricted RN licensure in the state where the candidate resides
Ability to obtain multi-state licensure
eNLC preferred or ability to obtain
Two to four years of clinical experience required
Hospital experience in medical-surgical, critical care, or inpatient care setting preferred
Experience in Emergency Room, EMT, or triage
Excellent verbal and written communication skills
Knowledge of health insurance, community resources, and community-based healthcare coordination
Excellent organizational and prioritization skills
Proficiency with computers, software applications, and phone systems
Knowledge of hospital electronic medical records (EMRs)
Manage patient information according to HIPPA guidelines
Ability to plan, supervise, and review the work of clinical and non-clinical staff
Ability to work under stressful conditions or irregular hours
