About the role
Key responsibilities & impact- Conduct patient outreach and engagement through video messaging, video consultations, phone calls, and text communication
- Educate and enroll patients in the medication adherence program
- Deliver personalized education, coaching, case management, and adherence support through video and digital communication
- Conduct video-based medication reconciliation consultations
- Review patient medication videos to verify administration, monitor adherence, and identify side effects or concerns
- Create short educational and motivational videos for patients and population-level health education
- Assess medical, behavioral, and social needs and escalate cases when necessary
- Provide guidance on medication administration, inhaler technique, chronic disease management, and medication safety
- Serve as a clinical escalation point for non-clinical team members
- Assist patients with technical troubleshooting of the mobile application and adherence platform
- Maintain accurate documentation in compliance with company and regulatory requirements
- Coordinate with provider offices, health plans, and pharmacies on behalf of patients
- Refer patients to healthcare providers and community resources, including warm transfers
- Use motivational interviewing to support long-term adherence strategies
- Contribute patient insights to program improvement
- Partner with cross-functional teams to develop video content, clinical protocols, and educational materials
Requirements
What you’ll need- Must be a legal resident of Puerto Rico
- Must hold a Registered Nurse (RN) license in good standing
- At least 5 years of nursing experience
- At least 5 years of related experience in patient engagement, health education, or care coordination
- Must be fluent in English and Spanish, including reading, writing, speaking, and understanding
- Strong background in chronic disease management, medication adherence, and patient coaching
- Excellent oral and written communication skills with a warm, empathetic approach
- Ability to work in a fast-paced, patient-focused environment
- Proficiency in electronic health records (EHR), mobile applications, and digital health platforms
- Comfortable engaging patients via phone, video, and chat throughout the workday
- Experience assessing social determinants of health and connecting patients with resources
- Must be available for evening and weekend shifts
- Must be legally authorized to work in Puerto Rico/United States without employer sponsorship; visa sponsorship is unavailable
