About the role

  • Provide long-term care management to frail, economically disadvantaged, culturally diverse older adults, disabled adults and caregivers.
  • Conduct in home and telephonic assessments, care planning, and client monitoring.
  • Coordinate linkages with community-based services and resources.
  • Maintain legal and clinical written record of each client’s case.
  • Ensure timely implementation of services for clients by completing all work within required timeframes.
  • Address client’s needs by developing and implementing a comprehensive care plan.
  • Ensure coordination of services and continuity of care by maintaining ongoing documentation.
  • Maintain effective communication with other disciplines working with clients.
  • Increase knowledge of care planner functions and actively participate in supervision.

Requirements

  • Bachelor's Degree in Social Work, Psychology, Gerontology or related field required.
  • Master's Degree in Social Work (MSW) preferred.
  • BILINGUAL- Must be bilingual in English/Spanish. (Test will be administered to assess proficiency.)
  • 1-2 years experience working with the elderly.
  • 1+ year experience working in a hospital, home health agency or related setting in which the primary targeted population is older adults.
  • Ability to travel within program’s catchment area and visits to client homes is required.
  • Strong organizational, verbal and written communication skills; Demonstrated customer focus; and Ability to work well with others required.
  • Travel 50+% of the time in Los Angeles and surrounding areas.
  • Must have a valid driver’s license, automobile insurance and reliable transportation.
