About the role
- Communicates with providers, case managers, coders, and healthcare team members to facilitate comprehensive medical record documentation
- Codes and reviews home health documentation
- Utilizes the organization’s designated clinical documentation system
- Provides or coordinates education related to compliance, coding, and clinical documentation issues
- Acts as a consultant to coders when additional documentation is needed
- Responsible for the evaluation of documentation by the medical staff and healthcare team
- Gathers and analyzes information pertinent to documentation findings and outcomes
- Contributes to a positive working environment
Requirements
- HCS-D coding certification experience required
- Current OASIS certification required
- Home Health experience is a must
- EMR: Homecare Homebase and Epic experience preferred
- If working in any service line other than Home Based Services (HBS): Bachelor’s degree in nursing required
- Graduate from a nationally accredited nursing program preferred
- If no degree, will consider seven years equivalent work experience
- Additionally, three years of recent clinical nursing experience required
- Experience in case management, utilization management, and/or coding preferred
- Currently holds an unencumbered RN license with the State Board of Nursing
- Certification in Clinical Documentation Specialist (CCDS) or Clinical Documentation Improvement and Integrity (C-CDI) preferred
- Remote work
