About the role

  • Responsible for the overall coordination of acute, urgent and residential care needs across Hesse Rural Health.
  • Oversees the planning, implementation and monitoring of clinical care and provides leadership and guidance to the multidisciplinary team in the delivery of high quality services.
  • Ensures a clinical focus on residents and patients when coordinating comprehensive care planning to maximise quality outcomes for patient, residents and outpatients.
  • Position is based at the Winchelsea campus of Hesse Rural Health and is responsible to the Director of Clinical Services.

Requirements

  • Current nursing registration with AHPRA
  • High level of clinical nursing abilities in a range of settings and dedicated experience in providing residential aged care and services
  • Sound knowledge and understanding of Aged Care Act 1997, the aged care quality standards and quality indicators.
  • Understanding of National Quality and Safety Health Service Standards
  • Demonstrated interpersonal and written communication and effective problem-solving abilities
  • Ability to work effectively with others and provide leadership within a team environment
  • Computer literacy including: the use of the Microsoft Office / M365; Microsoft Outlook and effective use of electronic databases.
  • Current Victorian Drivers Licence.
