About the role
Key responsibilities & impact- Build Referral Partnerships: Develop relationships with physicians, care coordinators, social workers, assisted living facilities, and senior-focused organizations.
- Own Territory Growth: Grow referral volume across your market by developing new partnerships.
- Operate in the Field: Spend 4–5 days per week meeting partners in person.
- Collaborate Cross-Functionally: Partner with clinical, operations, and patient experience teams.
- You're comfortable with range: You can give a presentation to 20 providers and walk into a solo PCP's office.
