Financial Planning and Analysis

• Maintain and update financial models to support breakeven analysis, funding discussions, operating plans, and long-range financial forecasts
• Incorporate membership trends, risk adjustment factors, and value-based reimbursement methodologies into models
• Analyze revenue, medical costs, membership trends, and profitability
• Prepare variance analysis across actuals, budget, and forecast with a focus on payor performance, capitation, and risk-based arrangements
• Support financial dashboard consolidation and development of KPIs tied to healthcare quality, STAR ratings, and cost savings
• Compile and analyze performance data from payor partners to track compliance with contractual terms and value-based metrics
• Partner with internal stakeholders to forecast medical costs, utilization trends, STARs performance, and quality outcomes
• Provide insights that inform negotiations and strengthen provider–payor relationships
• Support development of annual budgets and rolling forecasts, integrating membership growth and medical cost assumptions
• Collaborate with department leads to gather inputs and provide analytical support on financial trends and cost drivers

• Support month-end close processes, including journal entries, reconciliations, and accruals
• Assist in preparing internal and external financial reports with a focus on accuracy, compliance, and healthcare-specific reporting needs
• Work closely with clinical, operations, and data teams to understand the financial impact of care delivery, population health, and cost management strategies
• Support ad hoc analysis and reporting needs for leadership, investors, and external partners

Financial Analyst

Manager

• Implementing and reviewing healthcare plans for geriatric patients, those recovering from serious injuries, or dealing with chronic illnesses
• Collaborate with doctors and other medical professionals to provide comprehensive care
• Advocate for patients and coordinate their care across settings
• Create and manage plan of care for chronic or serious conditions (diabetes, heart disease, CHF, cancer, etc.)
• Schedule and follow up on hospital post-discharge appointments within 72 hours
• Communicate with patients and families about health conditions and care plans
• Offer education and guidance in collaboration with primary care physicians for complex medical decisions
• Serve as a liaison between patients and insurance providers to promote quality, cost-effective care
• Verify daily Sage Health patient hospital census and planned discharges
• Identify and provide resources and information to assist patients and family members (in collaboration with CMO/designee)
• Hold regular meetings and communications with clinical support staff, physicians, and operations leadership
• Assist primary care physicians in executing virtual appointments in patients' homes
• Ensure overall quality of healthcare and update electronic health records with recent treatments
• Maintain readiness for health plan audits, and local, state, and federal inspections
• Stay up to date on nursing regulations and support the clinical staff

Nurse Case Manager, RN

Security Engineer

• Patrol the exterior/interior areas of Sage Health centers to detect and deter security threats
• Monitor surveillance cameras, panic alarms and alert the authorities when appropriate
• Respond to emergencies, security incidents, and participate in life safety exercises
• Enforce Sage Health rules, regulations, policies and procedures
• Conduct security checks and inspections
• Write detailed reports on incidents and observations
• Serve as a member of the Sage Health Emergency Preparedness & Response Team

Unarmed Physical Security Specialist

Account Executive

• Develop, implement and execute monthly and quarterly sales plans to achieve new patient goals.
• Plan and execute local community events (tabletop events, health fairs, partnerships with local food banks) targeting Medicare-eligible seniors (typically 55+).
• Generate leads by prospecting and building relationships with seniors, community partners, contracted plan agents and independent brokers.
• Cultivate relationships with contracted health plan agents, independent brokers, senior housing leaders, senior recreation centers, local churches, and other community organizations.
• Educate potential patients on services at local events and through health agents; provide personalized tours of centers and introduce prospects to PCPs and staff.
• Deliver high level of sales and customer service to prospects, patients, health agents and community partners.
• Nurture ongoing relationships with patients to ensure satisfaction and retention.
• Attend regularly scheduled sales meetings (local, regional and/or national) and share sales plans and results.
• Report to Area Sales Manager and perform other related duties as assigned.

Healthcare Outside Sales Representative

• The Community Outreach Specialist is an outside healthcare sales representative responsible for generating sales leads of potential Medicare eligible Seniors and converting them to Sage Health patients.
• Nurture relationships with senior citizens, local community partners, contracted plan Captive agents and Independent Brokers.
• Create and manage community events (tabletop events, health fairs, partner with local food banks etc) to attract Seniors and drive leads into the funnel.
• Develop, implement and execute a productive monthly and quarterly sales plan to achieve new patient goals with support of local Sales Manager.
• Generate leads by prospecting, building and maintaining relationships throughout the community.
• Cultivate solid relationships with contracted health plan agents, independent brokers, senior housing leaders, senior recreation centers, local churches.
• Educate potential patients on services at events and through health agents, give personalized tours of center and introduce them to PCPs and staff.
• Deliver highest level of sales and customer service to prospects, patients, health agents and community partners.
• Continue to nurture ongoing relationships with patients that join Sage to ensure satisfaction.
