Case Manager
S.U.C.C.E.S.S.Case Manager providing support and case management to vulnerable immigrant families and youth. Involves recruitment, assessment, and collaboration with community agencies for settlement assistance.
About the role
Key responsibilities & impact- Provides recruitment through outreaching to at-risk/vulnerable immigrant and refugee families and youth
- Conducts comprehensive intake, eligibility and needs assessment
- Performs a case management approach through setting action plans as well as the "wrap around" service plan with clients
- Offers continuous case monitoring and follow up
- Conducts home visits when necessary
- Provides settlement counselling and settlement assistance and support to clients
- Bridges clients to different services
- Liaises and works closely with community agencies and partnering agencies
- Conducts life skills, literacy and essential skills group sessions for clients
- Provides group services, case conferencing, supported access and transition support to external services
- Ensures all services meet accreditation standards
- Maintains up-to-date case records
- Responsible for report writing and making case records
- Maintains statistical records for accountability, funding applications/reports and program development purposes
- Attend case conferences, supervision and team meetings regularly
- Attends division staff meetings and training opportunities as required
- Provides settlement assistance and support to newcomers arriving in YVR
- Works closely with staff members of other S.U.C.C.E.S.S. divisions/offices in providing joint support to individuals and families
- Performs other tasks as assigned by supervisor
Requirements
What you’ll need- Bachelor’s degree or Diploma in Social Work, Psychology, or related discipline
- Two (2) years of family/youth work experience and/or settlement work experience with vulnerable populations is preferable
- Experience in group facilitation
- Demonstrates knowledge about local community resources for at-risk and vulnerable immigrant populations
- Demonstrates experiences on conflict resolution, case management and basic counselling skills
- Demonstrates strong understanding of case planning and case management principles and techniques
- Demonstrates good knowledge of issues vulnerable immigrant families and youth are facing, including social, cultural, emotional, mental health, health and sexual orientation issues
- Demonstrates language and cultural proficiency that is applicable to the various source countries of vulnerable populations in their service area
- Demonstrates knowledge and experience in cross-cultural communication and service methods appropriate to clients with low literacy and low English skills
