About the role
Key responsibilities & impact- Provide support for individuals and/or groups of persons with disabilities in the community as part of waiver services
- Assist individuals in identifying and communicating requests and needs for services and supports
- Implement services and supports addressing each person’s goals and desired outcomes through their Centered Plan
- Understand individuals in the context of their personal history, social and family networks, and hopes for the future
- Promote healthy living and prevent illness and accidents
- Increase awareness of self-advocacy and support individual choice and decision-making
- Accompany people to work, movies, concerts, shopping, and other community outings
- Teach independent living skills
- Provide transportation to appointments
- Foster positive relationships within the community
- Promote decision-making about living, work, and social relationships
- Assist with personal care and bathing; help around the home with laundry, cleaning, and decorating
- Document the care provided
- Provide education on human, legal, and civil rights
- Use effective advocacy strategies for people living with disabilities
- Connect people with resources for career, education, and volunteer opportunities
- Promote effective communication
- Assist in medication administration where state law allows
- Protect confidentiality
Requirements
What you’ll need- High School Diploma or GED (Preferred (TN))
- At least 18 years of age
- Valid drivers’ license
- Conflict resolution skills
- Some computer skills preferred
- Decision-making skills
- Communication skills
- Ability to maintain good work attendance
- Ability to maintain confidentiality
- Complete criminal background screening
- Name checked in the OIG exclusions database, Child Abuse Registry, and Offenders Against Individuals with Developmental Disabilities registries
- Drug testing
- Education verification and other credentialing based on position requirements
- For positions requiring driving: proof of driver's license, driver's insurance, and vehicle, if required for transportation
