About the role

Key responsibilities & impact
  • Develop and implement treatment plans based on diagnostic and functional assessments approved by the clinical supervisor
  • Ensure compliance with Minnesota Rule Chapter 9520 and provide or ensure access to medical, vocational, social, educational, financial, legal, housing, and chemical dependency services
  • Develop an Individual Community Support Plan for each client every 6 months
  • Schedule face-to-face visits according to the ICSP
  • Make culturally sensitive referrals and assist clients in accessing needed services
  • Participate in commitment processes, including probate court testimony, court reports, stayed warrants, provisional discharge agreements, and pre-petition screening
  • File legally required reports with Vulnerable Adults, the Ombudsman, and Child Protection
  • Provide services that maintain and enhance clients' independent living abilities
  • Develop individualized emergency support plans with community emergency support services
  • Assist with housing issues and coordinate with HRA, PHA staff, and landlords
  • Attend quarterly reviews and treatment planning meetings
  • Collaborate with and educate family members and others involved in clients' treatment and welfare
  • Educate clients about symptoms, course, and treatment of their illnesses
  • Provide team and agency coverage of client needs
  • Develop therapeutic relationships with clients
  • Complete paperwork, reports, and charting contemporaneously and in an organized manner
  • Participate in ongoing professional growth, supervision, and appropriate professional conduct
  • Transport clients in the community to various appointments

Requirements

What you’ll need
  • Bachelor's degree in behavioral sciences or related field, or 30 semester credit hours in behavioral health sciences if no degree, plus 2,000 hours supervised work experience with individuals diagnosed with serious mental illness and/or substance use disorders
  • Master's or other graduate degree in behavioral health sciences or related field
  • If no degree, 4,000 hours supervised work experience with individuals diagnosed with serious mental illness and/or substance use disorders
  • Must have car, auto insurance and driver's license
  • Ability to work Monday–Friday, 8 am–4 pm or 9 am–5 pm, 40 hours per week
