About the role

Key responsibilities & impact
  • Provide Housing Support Services compliant with County, State, and Medicaid requirements
  • Assist clients in obtaining or maintaining housing, moving, and securing necessary furnishings
  • Help clients apply for, maintain, and understand benefits and complete related paperwork
  • Assist with budgeting and use of Housing Support funds as required by DHS
  • Teach and support basic independent living skills, tenant responsibilities, and renter rights
  • Help clients access community resources
  • Collaborate with property management, county financial workers, providers, and community members; advocate for clients and provide mediation
  • Maintain progress records and document clients' functional abilities and illness-management needs
  • Help clients create service-goal plans and provide instruction and monitoring according to person-centered plans
  • Monitor client progress, modify service plans, and communicate changing needs to the service team
  • Schedule and conduct client appointments in homes or the community
  • Intervene during emergencies and contact appropriate emergency services and support contacts
  • Coordinate care with agency and community providers and maintain release-of-information documents
  • Maintain compliance with Minnesota statutes and rules, including vulnerable-adult maltreatment, duty to warn, data practices, and HIPAA requirements
  • Meet regularly with the Program Director for client-needs reviews and treatment direction
  • Transport clients in the community

Requirements

What you’ll need
  • Bachelor's degree in Psychology or Social Work plus 2,000 hours of work experience providing services to adults diagnosed with a serious mental illness, OR no college degree and 4,000 hours of supervised work experience delivering services to adults or children with mental illness
  • Must have a car, auto insurance, and valid driver's license
