About the role

Key responsibilities & impact
  • Work with the larger treatment team to provide services for clients
  • Support development, writing, implementation, evaluation, and revision of treatment goals and plans
  • Develop individualized crisis plans coordinated with community emergency support services
  • Explain services, assess client needs, establish and implement care plans, and arrange resources for community transition
  • Coordinate services with county and community agencies, including Mental Health Case Management, Adult Protection, and community hospitals
  • Assist with comprehensive assessments, individualized treatment plans, substance abuse treatment, and other community-support tools
  • Assist clients with psychiatric rehabilitation and obtaining and maintaining housing
  • Teach or assist with independent living skills including cooking, shopping, cleaning, personal hygiene, transportation, and budgeting
  • Assist clients with appointments and recovery-process tasks
  • Attend commitment and other court hearings and complete court reports as requested
  • File Vulnerable Adult, Child Protection, Ombudsman’s Office, and other mandated reports when applicable
  • Engage clients’ family members/significant others, landlords, employers, and treatment team members with client permission
  • Provide on-call crisis intervention as determined by the Program Director
  • Complete paperwork, reports, and charting contemporaneously and in an organized manner

Requirements

What you’ll need
  • Bachelor's degree in Psychology or Social Work plus 2,000 hours work experience providing services to adults diagnosed with a serious mental illness, OR no college degree and 4,000 hours supervised work experience in the delivery of services to adults or children with mental illness
  • Ability to work Monday–Friday, 8 am–4 pm or 9 am–5 pm
  • Ability to provide on-call crisis intervention, including after-hours phone calls, as determined by the Program Director
  • Ability to complete paperwork, reports, charting, court reports, and mandated/required reports
