About the role
- Responsible for conducting comprehensive biopsychosocial assessments and evaluations for patients seeking SUD treatment.
- Conduct assessment through patient interviewing, data gathering, and reviewing standardized assessment tools.
- Utilize expertise in counseling or related field for assessment and recommendation of appropriate treatment and level of care.
- Facilitate initial treatment planning through patient collaboration on the development of prioritizing problems, goals and objectives, and interventions.
- Review treatment protocols, priorities, and expectations with patients.
- Ensure admission tasks, documentation, and consultations are completed as required during the day of service.
Requirements
- Active independent licensure in the state of Kentucky (LPC, LCSW)
- Strong verbal and written communication skills
- Ability to connect with patients through a telehealth platform
- Team oriented with clinical assessment skills and collaborative treatment approach
- Excellent computer skills and a demonstrated comfort with technology
- Excellent organizational skills
- 2 Year Substance Abuse/Addiction and/or Mental Health experience (preferred)
- Active independent licensure in the state of West Virginia or Ohio (preferred)
- Additional relevant certification - CADC (preferred)
