About the role
Key responsibilities & impact- Hybrid Clinical Manager leadership role supporting home health operations
- Coordinate patient care planning, referral reviews, and interdisciplinary case management
- Review new patient referrals for clinical appropriateness and approve/co-sign clinical orders
- Provide guidance to field clinicians regarding OASIS documentation and utilization review (No OASIS Review Caseload Required)
- Partner with physicians, therapists, schedulers, QAPI, and field staff to ensure regulatory compliance and continuity of care
- Assist the Director of Nursing with clinical oversight, scheduling support, and call rotation responsibilities
Requirements
What you’ll need- Active Registered Nurse (RN) License REQUIRED
- Minimum One Year Recent Registered Nurse Experience REQUIRED
- Minimum One Year Home Health Experience REQUIRED
- Thorough Knowledge of Medicare Conditions of Participation (CoPs) REQUIRED
- OASIS Documentation Knowledge REQUIRED
- Homecare Homebase (HCHB) Experience REQUIRED
- Valid Driver's License REQUIRED
- Completion of Candidate Attestation & Screening REQUIRED
- Leadership or Supervisory Experience Preferred
- Utilization Review and Quality Improvement Experience Preferred
