About the role
Key responsibilities & impact- Provide support to the Care Management team coordinating care for identified members/patients throughout the healthcare continuum
- Work closely as a healthcare team member
- Perform clerical tasks related to overall team functions and activities
- Support activities identified during the initial and ongoing assessment and management of members/patients
- Work under the direction of the Supervisor or Manager
Requirements
What you’ll need- Medical Terminology coursework or working knowledge of Medical Terminology
- 1 year of healthcare experience
- Associate's Degree in a related field (preferred)
- Medical Assistant Certification (CMA) upon hire (preferred)
- Managed care or case management experience (preferred)
- Experience with Epic (preferred)
