Requirements

What you’ll need

Bachelor's Degree from an accredited Physical Therapy Program.
Master's Degree from an accredited Physical Therapy Program.
D.P.T. from an accredited Physical Therapy Program.
National Provider BLS - American Heart Association within 30 days of hire.
Oregon Physical Therapist License upon hire.
1 year clinical experience in Home Health or healthcare setting, or active enrollment in the Providence postgraduate preceptor program.
