About the role

Key responsibilities & impact
  • Provides leadership and supervision of the day-to-day clinical and administrative operations of the care management team

  • Collaborates directly with inpatient and ambulatory clinicians and care teams

Requirements

What you’ll need
  • Washington Registered Nurse License upon hire
  • 5 years clinical nursing, including acute care hospital and/or ambulatory clinic experience
  • Bachelor’s Degree (preferred)
  • 3 years Case Management experience including care management, care coordination, discharge planning, or population health (preferred)
  • 2 years progressive supervisory experience (preferred)
