Key responsibilities & impact

Provides leadership and supervision of the day-to-day clinical and administrative operations of the care management team
Supervises Care Management (CM) staff including nursing, social work, patient care coordinators and other non-clinical staff
Collaborates directly with inpatient and ambulatory clinicians and care teams

Requirements

What you’ll need

Washington Registered Nurse License upon hire
5 years clinical nursing, including acute care hospital and/or ambulatory clinic experience
Bachelor’s Degree (preferred)
3 years Case Management experience including care management, care coordination, discharge planning, or population health (preferred)
2 years progressive supervisory experience (preferred)
