About the role
Key responsibilities & impact- Provides leadership and supervision of the day-to-day clinical and administrative operations of the care management team
- Supervises Care Management (CM) staff including nursing, social work, patient care coordinators and other non-clinical staff
- Collaborates directly with inpatient and ambulatory clinicians and care teams
Requirements
What you’ll need- Washington Registered Nurse License upon hire
- 5 years clinical nursing, including acute care hospital and/or ambulatory clinic experience
- Bachelor’s Degree (preferred)
- 3 years Case Management experience including care management, care coordination, discharge planning, or population health (preferred)
- 2 years progressive supervisory experience (preferred)
