About the role
- Operational owner for Post Acute programming across the North Division
- Develop and operate several programs that support patient throughput to post-acute settings
- Partner closely with external partners and entities
- Facilitate program implementation within case management teams and leadership
Requirements
- Master's Degree in Social Work
- 8 years relevant experience in Case Management (Care Coordination or Utilization Management)
- 1 year Project Management experience
- Upon hire: Washington Social Worker Independent Clinical License
