About the role
- Uses Case Management Guiding Principles in partnering with patients and physicians to assure access, services, and transition for the continuum of care.
- Works as team member with health care professionals, community resources, liaisons, and families to provide coordinated admission and discharge planning.
- Advocates and supports the case management process using the Physician-Centric Care Management Model as a guideline.
Requirements
- Graduation from accredited school of Nursing.
- Upon hire: Alaska Registered Nurse License.
- 2 years Clinical nursing experience.
- 3 years Clinical nursing experience (preferred).
- 1 year Utilization Management-review and Discharge-planning experience (preferred).
- Experience associated with healthcare professional and support staff training needs (preferred).
