About the role
- The RN Case Manager works collaboratively with patients, social workers, physicians, other practitioners, caregivers and community agencies in coordinating and overseeing the interdisciplinary plan of care for a patient in a home care/home hospice environment.
- Assesses, plans, facilitates and evaluates timely coordination of high quality, cost-effective care.
- Responsible for improving the health of patients dwelling in their own homes.
- This requires a nurse who is comfortable working independently, enjoys teaching, is a critical thinker and has strong communication skills.
- The RN Case Manager provides hands on care in the home environment and offers autonomy, flexibility and the luxury of caring for one patient at a time.
- Ideal candidates also possess negotiation skills along with strong analytical and computer skills.
- Case Managers also promote safety, cost efficiency and a commitment to quality improvement.
Requirements
- Associate’s Degree in Nursing required
- Bachelor's Degree in Nursing preferred
- Current State of WI or eNLC compact state RN license
- Valid State of WI driver’s license with an acceptable driving record
- Current BLS (CPR) certification or ability to obtain upon hire
- Clinical experience preferred
- EPIC software experience preferred
