Home Health RN Case Manager
Planned Parenthood Federation of AmericaRegistered Nurse (RN) Case Manager conducting assessments and providing care for home health patients at PPCCC. Collaborating on patient care plans and supporting postpartum health services.
About the role
Key responsibilities & impact- Responsible for initial and on-going assessments of Planned Parenthood California Central Coast Home Health patients assigned to their care
- Collaborate with the patient and the Home Health Manager to develop, implement, and evaluate the plan of care
- Provide visits prenatally to support patients in creating their postpartum plan
- Provide after-pregnancy care to patients
- Perform initial comprehensive and on-going assessments on assigned patients
- Support case management improvement with quality home health care
- Participate in orientation, competency testing, and in-services of all new personnel
- Develop and revise the plan of treatment with the patient and physician
- Document patient service activity in electronic health record and participate in audits
Requirements
What you’ll need- Possess a valid California driver's license and a dependable insured automobile
- Ability to travel within the PPCCCHH designated geographic area (Thousand Oaks to San Luis Obispo)
- Possess one year of professional nursing experience, postpartum experience preferred
- Graduate of an accredited school of nursing
- Possess and maintain a current active RN license issued by the California Board of Registered Nursing
- Maintain BLS certification
- Possess one year of experience with lactation support
- Proficiency with PMADs assessment
- English-Spanish bilingual skills required

Comp & perks- $1.00/hour bilingual Spanish/English differential pay is offered upon passing the language assessment test.
