About the role
- Conduct comprehensive assessments to identify client needs and develop tailored service plans.

Requirements
- Bachelor's degree in social work, psychology, or a related field preferred
- Minimum of 2 years of experience in case management or social services
- Knowledge of community resources and social service systems in Washington, DC
- Strong communication and interpersonal skills for effective client engagement
- Ability to work independently and as part of a collaborative team
- Proficient in case management software and documentation practices
- Compassionate demeanor with a commitment to client advocacy
- Valid driver's license and reliable transportation is preferred
