Key responsibilities & impact

Conducts brief behavioral health screenings and risk assessments.
Determines client eligibility and acuity.
Facilitates placement into appropriate programs or community-based services.
Collaborates with the Admissions Director and multidisciplinary teams.
Manages referral prioritization and tracks service authorizations.
Maintains accurate documentation within the electronic health record.
Partners with community agencies to provide care management, service planning, and ongoing coordination.

Requirements

What you’ll need

A confirmed bachelor’s degree in a relevant field in Counseling, Social Work, or related field is required.
One year of relevant experience in Behavioral Health and/or Case Management is preferred.
Bilingual (Spanish) preferred.
Valid driver’s license, auto insurance, willingness to travel to homes/communities.
Requires living in or near the location of the posting.
