About the role
Key responsibilities & impact- Identify, outreach, engage, and enroll individuals into the program virtually through phone, text, and email, and in person at clinic, community-based organization, and health system partner locations
- Listen openly to eligible members, meet them where they are, and conduct multiple outreaches as needed
- Educate partners about Pair Team’s services and link eligible members to the Enhanced Care Management program
- Explain Enhanced Care Management services and benefits to hesitant or distrustful members
- Build trust with eligible members and enroll them in the program
- Promote timely communication among internal teams and clinic, community-based organization, and health system partners
- Meet key enrollment metrics and support improved wellbeing of underserved communities
- Manage the end-to-end enrollment funnel and ensure members continue through the enrollment process
- Provide updates on funnel stages, delays, and blockers
- Use all additional internal and external technology tools required for the position
Requirements
What you’ll need- Physically located in the United States
- Virtual operations require a quiet, HIPAA-compliant, internet-connected space
- 1+ year of experience working in a metric- or quota-driven environment
- Dedicated to working within and improving the community you are part of
- Results-driven with a proven track record of exceeding performance indicators
- Bilingual English/Spanish with proficiency reading, writing, speaking, and listening in both languages
- Strong technical skills and comfort using multiple technology platforms
- Valid driver’s license and auto liability insurance
- Preferred: experience with high-volume outreach and engagement campaigns
- Preferred: reliability and comfort in an ever-changing environment
