About the role
Key responsibilities & impact- Identify, outreach, engage, and enroll individuals through phone, text, email, and in-person partner locations
- Listen to eligible members, conduct multiple outreaches as needed, and meet them where they are
- Educate partners about Pair Team’s services and link eligible members to Enhanced Care Management
- Explain Enhanced Care Management services and benefits
- Build trust with eligible members and enroll them in the program
- Promote timely communication among internal teams and clinic, community-based organization, and health-system partners
- Meet enrollment metrics and support improved wellbeing in underserved communities
- Manage the end-to-end enrollment funnel and ensure member continuation
- Provide updates on funnel stages, delays, and blockers
- Use all required internal and external technology tools
Requirements
What you’ll need- Physically located in the United States
- Quiet, HIPAA-compliant, internet-connected workspace for virtual operations
- 1+ year of experience in a metric- or quota-driven environment
- Dedicated to working within and improving the community you are part of
- Results-driven with a proven track record of exceeding performance indicators
- Bilingual English/Spanish proficiency in reading, writing, speaking, and listening
- Strong technical skills and comfort using multiple technology platforms
- Valid driver’s license and auto liability insurance
- Preferred: experience with high-volume outreach and engagement campaigns
- Preferred: reliability and comfort in an ever-changing environment
- Employment eligibility verification through E-Verify
- Passing a pre-employment background check, including criminal history, references, and driving records when vehicle use is required
