About the role
Key responsibilities & impact- Identify, outreach, engage and enroll individuals into the program virtually through phone, text and email, and in-person at clinic, community based organization, and health system partner locations
- Listen openly to eligible members, meet them where they are, and use multiple outreaches when needed
- Educate partners about Pair Team’s services and link eligible members to the Enhanced Care Management program
- Explain Enhanced Care Management services and benefits to hesitant or distrustful members
- Build trust with eligible members and enroll them in the program
- Promote timely communication among internal teams and external partners
- Meet enrollment metrics and support the mission of improving underserved communities’ wellbeing
- Manage the end-to-end enrollment funnel and support members’ continuation in the process
- Provide updates on funnel stages, delays, and blockers
- Use all required internal and external technology tools
Requirements
What you’ll need- Physically located in the United States
- Quiet, HIPAA-compliant, internet-connected space for virtual operations
- 1+ year of experience working in a metric- or quota-driven environment
- Dedicated to working within and improving the community you are part of
- Results-driven with a proven track record of exceeding performance indicators
- Bilingual English/Spanish proficiency in reading, writing, speaking, and listening
- Strong technical skills and comfort using multiple technology platforms
- Valid driver’s license and auto liability insurance
