About the role
Key responsibilities & impact- Oversee and supervise care of all residents
- Provide direct resident/patient care
- Oversee the health services department and coordinate resident care
- Manage health services systems and supervise health services team members
- Promote a positive physical and psychosocial environment for residents
- Complete and use level-of-care determination tools
- Assess residents’ health, functional, and psychosocial status
- Participate in individualized service plan development
- Proactively manage resident care and services
- Case manage resident needs and solve problems with the management team
- Evaluate residents with changes in condition
- Direct team members, plan services, and coordinate care
- Collaborate on prospective resident move-ins and conduct pre-move-in assessments
- Coordinate services with physicians, discharge planners, nurses, home health agencies, case managers, hospice, and state agencies
- Arrange ancillary services including pharmacy, podiatry, laboratory, and x-ray services
- Communicate with residents’ families and provide education and support
- Supervise and train health services team members in personal care and medication management
- Manage delegation of nursing tasks under applicable state laws and practice acts
- Investigate incidents and conduct root cause analyses
- Oversee documentation and communication of resident care and services
- Manage infection control training, investigations, reporting, and data tracking
- Maintain knowledge of rules, regulations, policies, trends, and standards
- Provide on-call support and phone triage
- Participate in leadership meetings, decision-making, incident follow-up, and community/company initiatives
- Represent the community to outside agencies and participate in community events
- Work within scope of license with RN oversight as needed
- Supervise and manage the entire Health Services staff directly or indirectly
Requirements
What you’ll need- Licensed practical nurse (LPN)
- Graduation from an Accredited School of Nursing with an LPN diploma
- Must be in good standing with the State Board of Nursing
- Must maintain all required continuing education/licensing requirements
- Proficient in Microsoft Suite products and Point Click Care (PCC)
- Must have a current unencumbered LPN license
- Ability to read, analyze, and interpret business periodicals, professional journals, technical procedures, and governmental regulations
- Ability to write reports, business correspondence, and procedure manuals
- Ability to present information and respond to questions from managers and employees
- Ability to apply fractions, percentages, ratios, and proportions to practical situations
- Ability to solve practical problems and interpret written, oral, diagram, or schedule instructions
- Ability to occasionally lift or move up to 75 pounds
- Prolonged use of a desktop or laptop computer
- Ability to sit, stand, walk, talk, read, and hear
- Travel by auto or airline may be required
