About the role
Key responsibilities & impact- Supports Adolescent & Young Adult Medicine clinic
- Coordinates health maintenance care for various patients
- Regular patient outreach via phone
- Assists with panel management and data collection
- Provides a range of medical assistant duties as needed
- Updates health maintenance and performs daily outreach
- Keeps referral log of patient communication
- Follows up on empaneled patients that have recently discharged
- Obtains medical records and proactively schedules appointments
- Participates in process improvement cycles and implementation of new workflows
Requirements
What you’ll need- Two years’ experience in a hospital or hospital-based clinic setting
- High School diploma or equivalent
- BLS certificate within 30 days of hire or prior to independent practice
- Completion of a nationally recognized accredited medical assistant training program, or a formal training program from the United States Armed Forces, or current Oregon Emergency Medical Technician (EMT) license, or current Oregon Practical Nurse License
- MA certification from a nationally recognized certifying body upon hire or completion of the evaluation period
- Ability to work independently and as part of a team
- Knowledge of patient flow and back-office functions
- Proficiency with EPIC, Microsoft Office including Excel, Word, and PowerPoint
