• Conduct thorough assessments to determine the mental health needs of cancer survivors and monitoring progress to adjust treatment plans as needed
• Administer evidence-based individualized and group survivorship therapy protocols to cancer survivors
• Collaborate closely with other members of the survivorship care team to identify and address patients’ mental health needs in the context of their individual history and unique experiences, promoting a holistic and integrated approach to survivorship care
• Support the implementation of an innovative cancer survivorship collaborative care program (CoCM), championing the integration of mental health support within the broader care model
• Support the care team in developing comprehensive care plans, providing expertise in behavioral health interventions where applicable to ensure holistic patient support
• Deliver psychoeducation to survivors and their families, utilizing evidence-based OncoveryCare educational content and cutting-edge resources on survivorship care

Survivorship Behavioral Health Clinician

• Conduct thorough assessments of cancer survivors via telemedicine to identify physical, emotional, and psychosocial needs
• Provide evidence-based guidance and education, collaborating closely with the survivorship care team to identify and address patients’ needs, considering their individual history and unique experiences
• Develop and update survivorship care plans in collaboration with patients, incorporating input from behavioral health professionals to support overall well-being
• Collaborate and communicate effectively with all members of patient internal and external care team, including oncologists, PCPs, and other specialists
• Provide education and support to patients regarding cancer survivorship, ongoing care needs, and available resources, using OncoveryCare’s evidence-based materials
• Work with clinical leadership to build and enhance survivorship care programs, incorporating patient feedback and best practices to improve patient experiences and outcomes
