Key responsibilities & impact

Assesses, plans, implements, coordinates and evaluates the nursing care of patients in their place of residence
Promotes continuity of care for patients on their assigned caseload
Provides direct patient care and supervises the care provided by LPNs and HHAs
Works in collaboration with all home health disciplines to provide care for patients
Applies principles of learning in teaching patient’s self-management and methods to regain independence

Requirements

What you’ll need

Associate’s degree required
B.S.N. preferred
Minimum of one year recent experience preferred in home health, acute or long-term care
Current Washington State Registered Nurse license, required
Current BLS certification, required
Valid Washington driver’s license, required
Proof of insurance on automobile used for work, required
Driving record in compliance with OMC standards required
