About the role

Key responsibilities & impact
  • Provide direct patient care for individuals enrolled in cardiac, pulmonary, and PAD SET rehabilitation
  • Complete intake evaluations, clinical assessments, exercise tolerance tests, documentation, and billing
  • Develop exercise prescriptions and progression for patients in all three programs
  • Manage cases, including individualized treatment plans, referrals, consultations, and communication with the Medical Director or referring physician
  • Circulate during exercise classes and document exercise, progression, vital signs, and other pertinent information in CareConnect
  • Teach and demonstrate strength-based exercises, including hand weights, body weight, and stretching
  • Monitor and troubleshoot continuous cardiac monitoring
  • Provide inpatient Cardiac Rehab teaching in the hospital during Phase I when needed
  • Coordinate clinical care through treatment plans, exercise prescriptions, and general healthcare coordination
  • Perform duties with focus on safety, quality, and efficiency as part of a High Reliability Organization

Requirements

What you’ll need
  • Master's degree in Exercise Physiology or related field
  • BLS – Basic Life Support certification from the American Heart Association
  • ACLS – Advanced Cardiac Life Support certification from the American Heart Association
  • 2 years of experience
  • Experience in cardiac, pulmonary, and/or SET for PAD rehabilitation
  • Internship in cardiopulmonary rehabilitation
  • High-level problem-solving, analysis, and interpretation of clinical problems
  • Excellent verbal and written communication skills
  • Active listening skills
  • Independent decision-making skills
  • Ability to manage stressful situations
  • Ability to respond appropriately to emergency situations
  • Ability to document clinical care in the medical record
  • ASCM or AACVPR certification desired
  • Experience with cardiac stress testing desired
