About the role
Key responsibilities & impact- Provide biopsychosocial assessments, psychiatric diagnoses, treatment plans, treatment interventions, therapy sessions, and discharge planning.
- Receive and manage referrals for at-risk populations from interdisciplinary care team members.
- Conduct patient, family, and significant-other interviews, chart reviews, and professional collaboration.
- Develop care plans and discharge plans incorporating the medical plan of care.
- Identify and refer cases of abuse, neglect, or exploitation to legally mandated authorities.
- Provide crisis intervention, information, referrals, and links to community resources.
- Deliver individual, family, or group clinical interventions, including supportive and educational counseling.
- Coordinate clinical and financial outcomes for assigned populations with interdisciplinary staff and external organizations.
- Monitor service quality, treatment-plan completeness, patient progress, and discharge planning.
- Document assessments and interventions in medical records and information systems while complying with regulatory requirements.
- Facilitate connections between patients and hospital or community services.
Requirements
What you’ll need- Master's degree in Social Work or a related field.
- LSW license in the State of Ohio.
- 1–2 years’ experience as an LSW, preferably in a patient-centric setting.
- Knowledge of medical terminology.
- Computer skills.
- Ability to provide care appropriate to the patient served.
- Knowledge of growth and development principles across different life cycles.
- Ability to act quickly and work in high-stress situations.
- Licensed Social Workers with a Bachelor of Social Work hired prior to 01/10/2019 are grandfathered into the job qualifications.
