Key responsibilities & impact

Works in partnership with patients, families, caregivers, and providers to deliver targeted care
Collaborates with interdisciplinary staff and stakeholders within the care continuum
Promotes patients’ optimal health through disease prevention and management strategies
Applies evidence-based interventions to populations for cost efficient and outcome-based care
Aims for positive outcomes including disease-based quality measures, patient satisfaction metrics, access to services, and hospital/ED utilization rates

Requirements

What you’ll need

Bachelor's Degree (Required)
APRN - Advanced Practice Registered Nurse License - State of Ohio Board of Nursing
BLS - Basic Life Support - American Heart Association
RN - Registered Nurse - Ohio Board of Nursing
2-5 years recent nursing practice experience with knowledge of acute hospital, community or public health nursing concepts and resources
Computer skill and data entry
Knowledge and skill in clinical assessment and disease management
Associates hired after 1/1/2020 must have their Bachelor’s degree in nursing.
