About the role
- Provides nursing care to patients and families along the health illness continuum in diverse health care settings while collaborating with the health care team.
- Identifies care gaps, follows up from HAI calls, assists with disease management, schedules appointments, and works through work queues.
- Performs initial, ongoing and functional health status assessment as applicable to the population and/or individual.
- Collaborates assessment findings with providers and members of healthcare team as appropriate.
- Delegates tasks in accordance with specific criteria and state statutes.
- Participates in development of self, peers and others, e.g. MAs, nursing students.
Requirements
- High School or GED (Required)
- BLS - Basic Life Support - American Heart Association
- LPN - Licensed Practical Nurse - National Council of State Boards of Nursing
- Must hold a current license from the State of Ohio.
- Successfully completed Ohio Board of Nursing approved medication course.
